  • Employment contracts signed by the nurse and healthcare facility (employer)
  • Financial declaration statement of the employer
  • Healthcare facility signatory's title, federal ID number, address, phone and fax numbers, and year of establishment
  • Nurse's high school and college transcripts (in English)
  • Birth certificate of nurse, and those of immediate family if applicable
  • Marriage Certificate (if applicable)
  • Valid Passport
  • Two fotos (2"x2")
